Normal Colposcopic Findings + Metaplastic Change

TRANSFORMATION TYPIQUE


Anatomie :

- C’est la cicatrisation NORMALE [1] d’un ectropion ou d’une ectopie. (Cf ces mots)

Les 2 Epithelia normaux, malmenés par une grossesse tendent à retrouver une situation progressivement normale sur le col

- Après la bataille la frontière retrouve sa place originelle.


Colposcopie :

  1. Le Col est rose à l’OEIL NU.
  1. L’endocol ne blanchit pas à l’ACIDE (c’est du Mucineux Originel)
  1. L’exocol brunit au LUGOL (c’est du Malpighien Normal Reconstruit)
  1. La Zone de Transition des Couleurs retrouve progressivement sa position à la frontière Exocol/Endocol, la ligne de front progresse toujours du vagin vers l’utérus, tendant naturellement à être attirée par son pôle géographique originel, la Jonction Anatomique.
  1. Seule la localisation de la Frontière entre les 2 mondes évolue.

- Cependant, sous l’oeil averti de l’archéologue, quelques fossiles enfouis sous la glace témoignent du phénomène de replatrage de l’exocol par le néo-malpighien sur cet ancien fond marin glandulaire.

- Les Glandes Uniques Fermées ou Kystes de Naboth

- Les Glandes Uniques Ouvertes ou Orifices Glandulaires Réguliers [2]

- Les Ilots Glandulaires (petites iles de "glandulaire normal" (rose et ne blanchissant pas, perdues dans un océan de Brun)

Métaplasie Mature
Orifice glandulaire unique non cerné,
Col Normal

- On parle parfois de Dystrophie glandulaire.


Conclusion :

- Le processus dynamique de la cicatrisation témoigne de la position stratégique du col entre 2 mondes que tout oppose.

- Lorsque la métaplasie est orthodoxe, les surfaces d’affrontement gardent les couleurs de la normalité.


Conduite à Tenir :

- La transformation Typique aboutissant à du tissu Normal, ne nécéssite aucune conduite particulière.

  • A. Original squamous epithelium

    Original squamous epithelium is a smooth, pink featureless epithelium originally established on the cervix and vagina. There are no remnants of columnar epithelium identified, such as mucus-secreting epithelium, cleft openings, or Nabothian cysts. The epithelium does not stain white after the application of a dilute solution of acetic acid ; it will stain brown after the application of Lugol’s iodine.

    B. Columnar epithelium

    Columnar epithelium is a single-layer, mucus-producing epithelium that extends between the endometrium cranially and either the original squamous epithelium or the metaplastic squamous epithelium caudally. At colposcopy after the application of acetic acid the area has a typical grape-like structure. Columnar epithelium is normally present in the endocervix and may be present on the ectocervix (ectopy) or, on rare occasions, in the vagina.

    C. The transformation zone

    The transformation zone is the area between the original squamous epithelium and columnar epithelium within which varying degrees of maturity may be identified. At different stages of maturity the metaplastic epithelium may stain slightly white after the application of acetic acid and partially brown after the application of Lugol’s iodine. Components of a normal transformation zone may be islands of columnar epithelium surrounded by metaplastic squamous epithelium—cleft openings and Nabothian cysts.
